Reporting incidents, accidents & hazardous situations

What if an incident, accident or near miss occurs?
Reporting and investigating incidents plays an important role in preventing similar events from reoccurring, and continuously improving our H&S system. Therefore, Vodafone is committed to establishing and maintaining an effective incident reporting, recording and investigation system. 

While contractors/partners must maintain incident/reporting/investigation process as part of their own H&S management systems, Vodafone partners are also required to:

  • Report all work-related accidents arising from Vodafone-related activities to the Vodafone Relationship Manager, Project Manager or Vodafone Health & Safety Manager as soon as practicable and within 24 hours of the incident occurring; and
  • Investigate causal factors of the incident and present a report to Vodafone.  For serious incidents, a formal investigation report to made available to Vodafone working 5 working days, or other incidents present a report to Vodafone within 15 days.

Minor hazards/ incidents issues should be tracked by the partner.

Vodafone managers may  request updates and review of ongoing performance as part of business-as-usual activities.

What if I see a hazard or unsafe situation?
If you see a situation that you consider is unsafe… Notify Vodafone ASAP and Report it!

This notification can occur a number of ways, depending on the nature of the issue and potential H&S risk that the issue creates. 

Facility issues
If the issue relates to a Vodafone facility, please log the issue with Vodafone’s facility management help desk on (02) 9415 7272 - Extension 3.

EME/RF issues
If the issue involves EME or RF transmission equipment, please click here for the process.

If the company you work for, or the partner that has engaged your company has a suitable hazard notification process, a Hazard Notification form can be completed and forwarded to the relevant Vodafone contact (i.e. the engaging partner, the VF project/relationship manager, or the Vodafone H&S team at AskHealthandSafety@vodafone.com)

As an alternative, the Vodafone Safety Corrective Action Request Form can be used.

Vodafone recognise that the systematic identification of hazards and management of Health & Safety related risks from our partners undertaking work on Vodafone facilities and networks is paramount to the Health and Safety of our employees, contractors and the public. Therefore, partners are also expected to conduct periodic inspections of work activities to ensure compliance with H&S guidelines.
